ProActive Exercise Referral Scheme

ProActive has been developed to introduce people to a more active lifestyle within a friendly, safe, enjoyable and supportive setting.

'ProActive' was developed by the Somerset Physical Activity Group (SPAG). SPAG is a countywide multi-agency group that develops strategies for the promotion of physical activity to all sectors of the community. Centres become accredited to the scheme after strict assessment for example two new centres joined Sedgemoor's scheme in April 2010.

Who is eligible to join ProActive?

You may be able to access the ProActive scheme if you have one or more of the following medical conditions:

  • Overweight/obese
  • Hypertension
  • Stable angina
  • CVA (stroke)
  • Claudication
  • Type 1 Diabetes
  • Type 2 Diabetes
  • COPD (mild severity)
  • Asthma
  • Epilepsy
  • Neurological conditions
  • Osteoarthritis
  • Rheumatoid Arthritis
  • Osteoporosis
  • Orthopaedic/Musculoskeletal
  • Anxiety
  • Depression
  • Other mild Mental Health issues
  • History of Falls
  • Fear of Falling
  • Cardiac problems
  • Any other medical condition where the severity is mild and the symptoms are stable.

How do I get referred?

Visit your relevant health professional (GP, Practice Nurse, Health Visitor, Physiotherapist, Dietician etc) and ask if they think you would benefit from some exercise. If they think you would, ask them if you could join the ProActive Scheme.

What happens next?

Your health professional will fill in a ProActive form and send it to the Somerset Primary Care Trust. It will then be distributed to the relevant District Co-ordinator for processing and shortly afterwards you will receive a phone call or letter detailing the courses on offer at various leisure centres.

Once you have made your choice, you will receive a phone call from the Co-ordinator at that leisure centre who will arrange a time to meet you for an initial consultation.

ProActive providers in Sedgemoor

  • Unique Health and Fitness Centre in Bridgwater - £54 for 8 weeks unlimited access
  • King Alfred Sports Centre in Highbridge - £46 for 16 sessions
  • Kings Fitness and Leisure in Cheddar - £20 initial consultation, £51.20 for 16 sessions
  • The Exchange Health Club in Bridgwater- £50 for 8 weeks unlimited access to specific sessions.
  • Burnham on Sea Swim and Sports Academy- £50 for first month (this includes Physiotherapy support) then either £40 or £27.50 depending on classes chosen.

Cardiac rehab phase 4 classes

  • Monday - Edington Village Hall - £2.50 per class - 9.40-10.55am.
  • Tuesday - Chilton Trinity Sports Centre - £3.50 per class - 9.15-10.25am.
  • Wednesday - Westonzoyland Community Centre - £3.50 per class- 10.00-11.15am
  • Thursday - Woolavington Village Hall - £3.50 per class - 10.00-11.15am
  • Friday - Bridgwater Pantomime Society - £3.00 per class - 9.30-10.45am
